The DCFS CSEC Reporting Forms facilitate the data collection for CSEC-identified youth served in DREAM Court in CLC. Case Managers and Attorneys must fill out the forms on a quarterly basis to track outcomes for education, mental health, substance use, employment, placement stability, and services the youth is has received. Below are the timeframes for the quarterly schedule and reporting forms are due 15 days after the reporting period concludes.

Instructions
- Every quarter, each firm must track all the CSEC clients served during the reporting period on a spreadsheet. The spreadsheet is simple. Download the “Case Template” here. We call this the “master list” which contains open and closed cases served during the three month period. The spreadsheet should only contain the columns listed in the case template.
- At the end of the quarter, you (or the attorneys working on the case) will fill out a data collection form online for each client you served on that spreadsheet.
- Note: Most of the questions on this form will ask you how the youth is doing at the time you are completing the form (i.e., now). Other questions will ask how the youth was doing during the reporting period (the past three month period). For example, "how many times did the youth runaway during the reporting period?"
- Once all data collections are submitted for each client, you will send Kristine the spreadsheet of the active/closed cases. Kristine will match the spreadsheet data with the data collection forms. We will follow up with your firm if there’s any data entry questions or errors.
Frequently Asked Questions
- Do we need to report on youth who ran away from care during the reporting period?
- Yes, all youth are required to have a completed form. If the youth was missing during the whole reporting period or ran away at any point in time during the reporting period, a form should be completed. We would need to reporting the number of times the youth ranaway and the number of days. If the youth ranaway and returned on the same day, record that event as 1 time and 1 day.
- What if the case closes in during the reporting period?
- Yes, a form should be completed. The data should reflect the characteristics of the youth at time of case closure.
- Do we report on NMD youth?
- Yes, a form should be completed for al NMD youth in DREAM Court and those served by the CSEC Unit. NMD clients who are unhoused or in shelters will be asked additional questions on the data collection form.
