  • CSEC DCFS Reporting Form: DCFS Reporting Forms inform our quarterly DREAM Court Grant reporting for DCFS. The forms facilitate the collection of data for CSE-identified youth served in DREAM Court. Case Managers and Attorneys fill out the forms on a quarterly basis to track outcomes for education, mental health, substance use, employment, placement stability, and services the youth is has received. 
  • EPY Youth Data Form: Staff will complete forms when they are notified a youth is pregnant and when their clients are parenting.
